If we are provided with the necessary information, claims for hospital and surgical services will be submitted to your insurance for you. If we do not have all of the information, the claim will be sent to you and you will be responsible for submitting the claim to your carrier for payment.

Even though your insurance policy may cover a portion or all of your medical expenses, the responsibility for payment of fees is the obligation of the patient or responsible party. Your health insurance is a contract between you and your insurance company; therefore it is your responsibility to contact your carrier if there are questions with your claim. Coverage varies widely and we cannot predict or guarantee what part of our services will or will not be covered by your insurance plan. Failure to notify us in advance of applicable insurance coverage may result in your being directly responsible for the fee regardless of insurance contract.
