1. What can I expect on my first visit with your physicians?
Your initial office appointment is for a consultation with Dr. Conter and his staff. At that visit, we may make recommendations for additional tests or procedures, if necessary, to provide you with your care.
2. What Should I bring to my appointment?
If you have had any tests or procedures pertaining to the condition or illness we are seeing you for, please discuss this with our appointment secretary when you make your appointment. We will do our best to try and obtain all necessary information from your referring doctor. In some cases, we may ask that you bring the necessary test results or X-ray studies (usually copied on a CD disc) with you to your appointment.
On occasion, we may request that you obtain additional tests prior to your appointment with our staff. This will help to make your visit with us as informative as possible and often prevents an additional visit. We realize your time is valuable.
At check-in please provide the following: Drivers License or a valid picture ID, Insurance card(s), and a Referral letter/number (if required by your insurance).
Tests and information necessary for your visit: X-ray studies on a CD disc, (CT scans, Mammogram's, ultrasound, MRI) laboratory tests (recent blood tests), relevant records from your referring doctor or emergency room visit.
3. Do I need to do anything ahead of time to prepare for my appointment?
Prior to your appointment, please fill out the Personal Medical History forms, Registration forms, and Privacy Notice (HIPAA) forms. If you have time, please review our Office Fees and Policies before your visit. If you are not able to access these forms we will ask you to complete and review them at your appointment.
4. When will my surgery be scheduled?
Typically, your surgery will be scheduled as soon as possible after your appointment, depending upon medical priority and scheduling availability. We always try to schedule a date that best accommodates your needs, unless you're medical or surgical condition warrants immediate attention. You can assist in this process by knowing in advance dates that you are not available for your surgery and communicating these dates to our staff so that we may avoid scheduling conflicts.
5. Am I going to need to pay anything at the time of the visit?
Payments, Co-Pays, are due at the time the services are rendered. Payments for any office procedures will be required at that visit. Please refer to our Office Fees and Policies document for a detailed explanation.
